Neither company has admitted any wrongdoing. To resolve the litigation, ConnectOnCall and Phreesia have agreed to a $4.95 million class action settlement. Who Qualifies

• You may be eligible to participate in this settlement if:

What You May Receive

Eligible class members may receive the following benefits:

• Up to $5,000 in cash compensation for documented losses resulting from the alleged data breach. Claims at this tier must be supported by non-self-prepared documentation (e.g., receipts, third-party records).
• Up to $75 as a pro rata cash payment for class members who did not experience documented losses. The actual amount may vary depending on the total number of valid claims filed.
• Two years of dark web and medical data monitoring through CyEx Medical Shield Complete, valued at an estimated $360 per class member.

Actual payout amounts are not guaranteed and will depend on the number of valid claims submitted and final court approval.

How to File a Claim

2. Determine your claim type — decide whether you are filing for documented losses (up to $5,000) or a pro rata cash payment (up to $75), based on your circumstances.
3. Complete the claim form accurately and in full. You will be submitting your claim under penalty of perjury, so ensure all information provided is truthful and correct.
4. Gather required documentation if filing for documented losses. You must include non-self-prepared supporting documentation such as receipts, bank statements, or other third-party records. Self-prepared documents are not accepted for this tier.
5. Submit your claim by November 2, 2026. Claims submitted after this deadline may not be considered. Note that the deadline to object to or exclude yourself from the settlement is October 19, 2026, and the final approval hearing is scheduled for November 17, 2026.
